Many maternal complications occur after hospital discharge. Our program provides personalized support focused on the mother's physical recovery, emotional well-being, and confidence in advocating for her health during the fourth trimester. Our goal is to reduce preventable postpartum complications by ensuring mothers receive continued support and early identification of warning signs after hospital discharge.
We believe no mother should feel unheard, unsupported, or unsafe during the postpartum period.
Prenatal period: Approximately 31-33% of maternal deaths
Day of delivery/intrapartum: Approximately 16-17% of maternal deaths
Postpartum period: Approximately 50-52% of maternal deaths
The postpartum period deaths break down further:
Early postpartum (1-7 days): ~18-20%
Later postpartum (8-42 days): ~12-15%
Late postpartum (43-365 days): ~18-20%

Our MOM CARE process empowers women to beat these odds:
STEP 1
Receive a referral for Mom from a family or one of our partner Health Care Providers (HCPs).
STEP 2
Visit with Mom in the early postpartum period to assess her and Baby's progress (within 2 weeks).
STEP 3
Perform a comprehensive physical and mental health assessment. Educate Mom, provide services, personal care supplies, and referrals as needed.
STEP 4
Be a liaison between Mom and her HCP. Support Mom by advocating, empowering and educating for a year or more.
