Personalized Care, focused on Mom, after giving birth
Personalized Care, focused on Mom, after giving birth
We are a non-profit dedicated to maternal care and advocacy. Our Mission is to reduce maternal death by providing inclusive healthcare to all women during and after pregnancy. Our Services, known as MOM CARE, include advocacy, counseling, and education for pregnant, postpartum, and women affected by pregnancy or infant loss. We specialize in the postpartum period and emphasize healing for the mind, body, and spirit ... the whole person.
Prenatal period: Approximately 31-33% of maternal deaths
Day of delivery/intrapartum: Approximately 16-17% of maternal deaths
Postpartum period: Approximately 50-52% of maternal deaths
The postpartum period deaths break down further:
Early postpartum (1-7 days): ~18-20%
Later postpartum (8-42 days): ~12-15%
Late postpartum (43-365 days): ~18-20%
Here are a few reasons why this is happening:
Chronic Health Conditions
Delayed Recongnition of Complications
Inadequate Access to Care
Racial and Ethnic Disparities
Mental Health and Substance Use Disorders
Healthcare System Fragmentation
Provider Training and Workforce Issues
Delayed Postpartum Follow-up
Quality Care Concerns
Social Structure Factors
Our MOM CARE process empowers women to beat these odds:
STEP 1
Receive a referral for Mom from a family or one of our partner Health Care Providers (HCPs).
STEP 2
Visit with Mom in the early postpartum period to assess her and Baby's progress (within 2 weeks).
STEP 3
Perform a comprehensive physical and mental health assessment. Educate Mom, provide services, personal care supplies, and referrals as needed.
STEP 4
Be a liaison between Mom and her HCP. Support Mom by advocating, empowering and educating for a year or more.
